Martinez Boxing Houston — Waiver, Release & Assumption of Risk

I, the undersigned, acknowledge that boxing and physical fitness activities involve inherent risks of injury. I voluntarily assume all risks associated with participation in training, sparring, conditioning, and related activities at Martinez Boxing Houston, located at 3307 Texas Street, Houston, TX 77003. I certify that I am physically fit to participate and will notify staff of any medical conditions. I release Martinez Boxing Houston LLC, its owners, coaches, volunteers, and staff from any and all liability for injuries or damages arising from participation, except in cases of gross negligence as defined by law.
