Release of Liability / Martinez Boxing Houston
I, the undersigned, understand that boxing is a high-impact combat sport involving inherent risks of serious injury, including but not limited to: broken bones, concussions, cuts, bruises, sprains, strains, and in rare cases, permanent disability or death.
I voluntarily assume all risks associated with training at the 20,000 sq ft Martinez Boxing facility located at 3307 Texas St, Houston, TX 77003. I understand that training involves physical contact, use of equipment, and exposure to physically demanding exercises.
I hereby release, waive, discharge, and covenant not to sue Martinez Boxing LLC, David Martinez, all coaches, trainers, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me while participating in any activities at this facility.
I acknowledge that I am physically fit and have no medical condition that would prevent my full participation in boxing training activities. I agree to inform the coaching staff of any injuries, illnesses, or medical conditions that may affect my ability to train safely.
I grant Martinez Boxing LLC permission to use my image, likeness, and voice in photographs, video recordings, and other media for promotional and marketing purposes without compensation.
I have read this release of liability and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTOOD, AND AGREE TO THE TERMS OF THIS LIABILITY WAIVER.
